on the processing of personal data on the basis of Decree No. 520/20 Coll. of 8 December 2020 on the implementation of adaptation-integration courses
I hereby grant the Employment Agency HOFMANN WIZARD, s.r.o., with its registered office at Prešovská 201/4, Plzeň, Postal Code 301 00, ID No.: 252 36 016, registered in the Commercial Register of the Regional Court in Plzeň, Section C, Insert 11501, e-mail address: plzen@hofmann-personal.cz (hereinafter the “Controller”), my consent to the processing of my personal data:
1. Personal data to be processed:
name and surname,
date of birth,
citizenship,
number of
the travel document, date of validity of the document
district,
city-part of the place of reported residence,
data on work permit,
visa
e-mail address, telephone contact.
2. Purpose of processing
· Ensuring the participation of an Data subject, employee-a foreigner in the non-public part of the adaptation-integration course organized by the Center for the Integration of Foreigners under the auspices of the Ministry of the Interior.
3. The processing time for personal data is:
· On the basis of the consent for a maximum period of one year or until the revocation of the consent by the subject.
4. Personal data may be provided to the following third parties for further processing by the controller or for the purpose of performing legal duties:
Center for the Integration of Foreigners, Ministry of the Interior, or
other state authorities active in this area.
Parent company as
a provider of secure data storage.
· Court for the
purposes of court proceedings, authorities active in criminal proceedings
for the purposes of criminal proceedings, administrative authorities for
the purposes of administrative proceedings, lawyers, notaries in the
provision of legal services.
5. Legal basis
Article 6 (1) (a) GDPR-Consent
Decree No. 520/2020 Coll. of 8
December 2020 on the implementation of adaptation-integration courses set
by the Ministry of the Interior pursuant to Section 182 para. h) and i) of
Act No. 326/1999 Coll., on the stay of foreigners in the territory of the
Czech Republic and on the amendment of certain acts, as amended by Act No.
176/2019 Coll., (hereinafter the “Act”) to implement § 155b par. and 5
to 7 of the Act.

7. Personal Data Controller rights information:
In accordance with the provisions of Article 13 of Regulation (EU) No 2016/679 of the European Parliament and of the Council of 27 April 2016, the General Data Protection Regulation (hereinafter "the Regulation"), the Personal Data Controller informs that:
· the Personal Data Controller has not appointed a Data Protection Officer;
· the Personal Data Controller does not intend to transfer the personal data of the employee to a third country, an international organization or other than the aforementioned third parties;
the Data subject has the right, in accordance with the Regulation, to require the Personal Data Controller to access his personal data, to correct or delete them, or to limit processing, and to object to the processing, has the right to transfer such data to another controller, as well as the right to file a complaint with the Office the protection of personal data, if it considers that the Personal Data Controller proceeds in the processing of personal data in violation of the Regulation.
